This ECUA form should only be used if you are requesting an onsite sewage treatment disposal system (septic tank) permit with the Florida Department of Environmental Protection (FDEP). This form is designed to gather information required to determine sewer availability based on Florida Statute ss. 381.0065-381.0067. Please fill out all sections accurately.

Definition of "Available" Sewerage System (Florida Statute 381.0065-381.0067):

A publicly owned or investor-owned sewerage system is "Available" if it is capable of being connected to the plumbing of an establishment or residence, is not under a Department of Environmental Protection (DEP) moratorium, and has adequate permitted capacity to accept the sewage to be generated. Additionally:

For Residential Subdivision Lot, Single-Family Residence, or Establishment (estimated sewage flow < 1,000 GPD): A gravity, low pressure, or vacuum sewer collection line exists in a public easement or right-of-way that abuts the property line.

For Establishment (estimated sewage flow > 1,000 GPD): A sewer line, force main, or lift station exists in a public easement or right-of-way that abuts the property or is within 50 feet of the property line (via existing rights-of-way/easements).

For Proposed Residential Subdivisions (> 50 lots) & Proposed Commercial Subdivisions (> 5 lots): A sewerage system exists within one-fourth mile of the development (via existing easements/rights-of-way).

For Repairs/Modifications in Industrial/Manufacturing Zones: A sewerage system exists within 500 feet of an establishment’s or residence’s sewer stub-out (via existing rights-of-way/easements).
